Perinatal SUD Resources Map

The Missouri Department of Health and Senior Services, MO HealthNet, the Missouri PQC, the Missouri Hospital Association, and The Uplift Connection have partnered together to create a statewide tool designed to connect healthcare resources for perinatal substance use disorder (SUD) and improve community health across Missouri. They are asking for help in expanding their Perinatal SUD Resources Map to help support Missouri's mothers and infants.


By sharing your organization's services, you can help improve access to care and coordination for families. Your approval to publicly share the following details is required:

  • Provider name, location, phone number, and website

  • Services offered

  • Accepted insurance/payment types


If your organization is already listed on the map, but have updates, please email info@theupliftconnection.org


If your organization is not on the map, please complete their new provider survey on The Uplift Connection SUD Resources pages, which can be found here.


This survey should be completed by a lead supervisor with knowledge of your program. Please complete and submit by May 1, 2025.
