Participate in upcoming Virtual Focus Group focused on improving postpartum care transitions

The St. Louis Integrated Health Network (IHN), in partnership with the EleVATE Collaborative, is inviting clinical team members across Missouri to participate in upcoming virtual focus groups focused on improving postpartum care transitions and we were hoping that you would forward this invitation to your colleagues in the Bootheel.


IHN is currently investigating how postpartum individuals transition from obstetric care to primary care during the 12 months following birth. Through these focus groups, we aim to learn directly from postpartum care team members about effective strategiescommon barriers, and opportunities to strengthen continuity of care after delivery. 


Virtual Focus Group Dates (CT): 

·       January 22 at 12:00 PM 

·       January 26 at 12:00 PM 

·       February 2 at 9:00 AM


Currently recruiting participants who provide direct patient care or oversee programs serving postpartum individuals in the State of Missouri.


Participants will receive a gift card in appreciation of their time.


A Zoom link will be provided upon registration. 


In addition to the focus groups, the team is conducting a brief statewide needs assessment survey to better understand how birthing people are supported during pregnancy and the postpartum period, including care coordination, referrals, and transitions across systems. The survey includes 30 questions and takes approximately 12 minutes to complete. 


Findings from both the focus groups and survey will be used to identify actionable gaps and promising practices, and to inform the development of training materials, referral pathways, and policy recommendations to improve postpartum continuity of care across Missouri.
